Steps in securing NSO-Civil Registry Documents
STEP 1
Fees:
Birth/Marriage/Death Certificate: Php140.00/copy
CENOMAR: Php195.00/copy
STEP 1
- Get the desired application form from the forms dispenser located at the lobby and fill up the required information.
- Secure a priority number from the security guard.
- Prepare right away an exact amount corresponding to the document you are requesting.

B. Legitimation
C. Advanced Endorsement/Submission
D. R. A. 9048 (Petition of Clerical Error/Change of Name)
E. R. A. 9255 ( Authority to Use the Surname of the Father)
F. Supplemental
G. R. A. 9858 (Legitimation of Children born below marrying age)
H. Court Order (ex: correction of date of birth, gender, annulment, divorce, etc.)
Please inform the CASHIER/COLLECTING OFFICER immediately.
STEP 2
- Wait for your number to appear on the queue number display. Then, proceed to Window 1 or 2.
- If your requested document has undergone the process of the following:
B. Legitimation
C. Advanced Endorsement/Submission
D. R. A. 9048 (Petition of Clerical Error/Change of Name)
E. R. A. 9255 ( Authority to Use the Surname of the Father)
F. Supplemental
G. R. A. 9858 (Legitimation of Children born below marrying age)
H. Court Order (ex: correction of date of birth, gender, annulment, divorce, etc.)
Please inform the CASHIER/COLLECTING OFFICER immediately.
- Wait for the issuance of your Official Receipt (OR).
- Before leaving the counter, check if the information indicated in your OR are complete and correct.
- Otherwise, immediately request the cashier to make necessary correction on your receipt.
- Wait for your name to be called by the Releasing Officer.
STEP 3

- When your NAME is called, proceed to Window 3 or 4 and present your Official Receipt (OR) for releasing of your requested document.
- IMPORTANT: If you are NOT the owner of the document, the parents, the spouse, or the direct descendant, please bring the following requirements:

STEP 4
- If you have inquiries/clarifications regarding the result of your requested document, ask the Releasing Officer or you can proceed to the Public Assistance and Complaints Desk/Outlet Supervisor/Outlet Manager.
_ IMPORTANT
NOTICE:
1. If you were not able to claim your requested document on the scheduled date of release, just proceed to Window 3 or 4 and present your OR and the needed requirements to the Releasing Officer. Provided, that the document you are claiming did not lapse a period of 30 working days from the date of filing.
2. Please do not transact with FIXER.
3. The Serbilis Center is open even at noon time.
